Scheduling your first appointment with one of our doctors is easy.  Here you will find all the necessary new patient intake forms and learn what to expect on your first consult.  Please check out the other pages on our site to learn more about our practice and how we can help you achieve your health goals.

There are just a few steps involved to become a patient at our clinic.

CALL US at 805-687-0533 to schedule a consultation and exam. If you email or call on Friday or a weekend, we will get back to you on Monday morning.

Schedule Your Appointment with Our Staff

When our staff contacts you we will schedule your initial consultation and exam and let you know which forms you should fill out from our website. We’re open Monday through Wednesday, 9 a.m. to 6:30 p.m. and Thursday, 9 a.m. to 6 p.m.

Fill Out Your Paperwork

When doing this, it is best to set aside 30-45 minutes where you will be free of distractions.  Our ability to draw effective conclusions about your state of health and how to optimize it’s improvement depends largely on the accuracy of the information in which you provide, including symptoms that you may consider minor.  Once complete, please return your intake forms to our office no later than 2 days prior to your appointment.

Your Exam

During your initial office visit the doctor will perform a comprehensive history and exam. Depending on your condition, this exam may include neurological, orthopedic, nutritional and/or kinesiological testing. If necessary, we will also order X-rays, bloodwork or other specialty testing on this visit and complete the appropriate in-office assessments.

New Patient Intake Forms

These forms are designed to help us get to the cause of your current health problems as quickly as possible. The more detailed and accurate you are, the better care we can provide. Your overall health is just as important to us as your current major complaints. No symptom is insignificant. The more you share with us, the more we will be able to help you achieve your health goals.
